GRANT INFORMATION

This section  gives information on the grant that we have been approved for.

Our Council will consider applications from those local entities that provide community "public outreach" services that need auxiliary aids or services to assist those with disabilities in meeting the challenges of architectural, transportation and communication barriers.

Any money awarded must be used to ensure that individuals with disabilities have equality of opportunity, allow them the ability for full participation in community events and services, and promote self-sufficiency in the full participation of activities that advance their overall quality of life.

Applicants must submit materials postmarked no later than October 25, 1999. If you wish to receive a confirmation that we have received your application, you must include a self-addressed envelope with your application materials.

Disbursements typically will range from $500.00 upwards until the amount allocated under this program has been depleted, money will be used; how many people it will reach; and the quality of your request.

Please note that incomplete applications will not be considered.

ELIGIBILITY STANDARDS

Organizations are eligible for support only if they:

• Your organization must provide community services to the residents residing within 5 miles of the Council's address as of July 30, 1999 (20 Hicksville Road, Bethpage, NY).
• Additionally, the entity must provide year-round services.

Applications will not be accepted from:

• Religious or other membership organizations providing services mainly to their own constituencies. The services offered must be available to all community members irrespective of religious or organizational membership requirements.
• Organizations requesting funds for scholarship funds or other short-term projects.

DESCRIPTION OF PROGRAMS

Promote the elimination of physical barriers for those that have physical disabilities in our surrounding community.

Our council agrees with the findings of Congress that some 43 million Americans have one or more physical disabilities. These individuals continually encounter various forms of unfair challenges as the result of the effects of architectural, transportation, and communication barriers that those that do not have physical disabilities do not currently face. Disability to us means, with respect to an individual's physical impairment, a factor that substantially limits one or more of the major life activities of the individual.

APPLICATION FORMAT

Applications should be stapled together. Do not insert the applications into binders or folders. Only one copy of the application is necessary. Incomplete applications will not be considered.

Organization Information and Accomplishments (no longer than 2 pages)

• Brief description of organization's history and mission.
• Current community outreach programs or activities. Please be specific as to how your programs meet the needs of the disabled and/or how the funds if awarded, will help meet the needs of those people that are disabled that may need to gain access to the community services you provide.
• Names, titles, and years of service of senior staff members.

  Request Purpose

      Please note that our Council may not fund the entire cost of a request.

• A statement as to why you need that money and how the money will be used. Please be specific as to what physical barrier will be addressed (Requests must address the need for the following aids and services: (a) taped texts or other effective methods of making visually delivered materials available to individuals with visual impairments; (b) the acquisition or modification of equipment or devices to make the facility more readily accessible to the disabled (e.g., modification of architectural and communication barriers, TDD devices for the deaf, etc.); (c) other similar services and actions that aid the disabled).
• The population/community members you plan to serve and how this population will benefit from the award.
• Strategies you will emploiy to implement your project should any money be awarded.
• Project budget and/or cost for and potential expenses incurred to implement your project and what portion of the budget or overall cost would any award granted by our Council be allocated to.
