Name: Your Name or the one you want the poem to go up as under
Name of Poem: The name of your poem
Email: if you would like people to be able to email you tell me and I will link your name
Background and text color: If you want it to look a certain way type how here and I will be glad to ablige. Also if you have a perticular backround just send it as an attachment.
Dedications: if you want to dedicate it to someone special or just for the fun of it.
Brief note: If you would like to say a little somehting about your self. Like why you wrote your poems or what they mean to you. I will place a brief note on your opening page.
Others:Anything else you feel should be up there with it.
