Dear Helena:

We have attempted to contact you numerous times regarding your overdue Pap smear. When we discussed the option of follow-up Pap smears versus cryotherapy of your cervix, we also discussed the possible consequences of ignoring cervical dysplasia. Although we discussed the possibility that cervical dysplasia can spontaneously regress, it also has the possibility of progressing, ultimately to cancer. The importance of continued and close follow-up has been emphasized for that reason.

If you have chosen to follow this problem with another physician, please contact me so that your records can be forwarded. Otherwise, I cannot overstate the importance of recieving follow-up Pap smears. I would encourage you to contact our office as soon as possible in order to receive this very important follow-up care. We understand that schedules are very busy these days; however we are here Monday through Friday and are willing to accommodate difficulties with scheduling.

If you have any questions, please feel free to contact us as anytime, and we look forward to hearing from you soon.

Sincerely,
Paul I. Berkowitz, MD
