You may print it out and mail it in with your contract, demo (the more copies you send the better) and picture; OR you may choose to do it on-line. {If you choose to do it on-line, please copy and paste the application in microsoft word, fill it out and e-mail it as an attachment to JupitersRaindrop@yahoo.com} HOWEVER you will still have to snail-mail me the contract, demos, and pictures!
