Key Responsibilities-Future Goals:  
Presently work as a RN Supervisor  at a sub-acute facility.  Tasks include supervise and management of staff; client care assessment /assist with est. goals (nursing and client).  Skills include Infusion therapy, lab interpretation values and  instituting/maintaining  medical regimen. Future goals include continuing education in medical coding accounting and medical management of client population through teaching assessment via computer skills.





Work Information and Brief Job History:


I currently work as a RN supervisor full time, in which responsibilities are assessment of patient status, evaluating patient care, staff scheduling and evaluation.  This includes medical orders, requisitioning of labs and interpretation with appropriate follow up of any changes in pt. status and outcomes.  ( This includes ventilator dependent patients and sub acute care).  Scheduling of staff and pt status changes.
Recently I have worked  as a RN staff nurse- charge nurse on a skilled sub acute unit. Responsibilities included, assessing of patient rehabilitative goals, medical treatments, reporting clinical changes of improvement or decline with appropriate intervention, which may include transfer to an acute care facility. That facility had a skilled unit of 24 beds which are sub-acute and 30 non skilled care. Although there were permanent residencies many were 30-60 day pt. stays that include IV's, postoperative surgical care and rehabilitative therapies in which the RN integrates with other disciplines.
I have also worked as a  staff nurse on a 32 bed sub acute care ventilator unit. This unit also had telemetry capacity for all rooms with 6-8 pts on EKG monitoring at any given time. Basic ekg/telemetry knowledge was required. Experience was gained with ventricular assist devices for potential heart transplant patients, RN responsibilities included monitoring of pt left / right vad volumes every 4 hours.
Peritoneal dialysis patients were also a part of the patient population on this unit; responsibilities were to maintain accurate daily weights for scheduled peritoneal dialysis and to initiate/engage hookup to peritoneal dialysis. Due to the nature of vent dependent patients experience with common respiratory treatments, tracheotomy care and collaborative work with respiratory therapists was essential on this unit.

Agency Nursing

I also worked as an agency nurse in long-term and med-surgical facilities. I was staffed on sub-acute units that require skills most comparable to a medical-surgical unit in terms of acuity, care planning, meds and treatment regimes. In addition note that I have worked concurrently for 2 agencies, although at times difficult, this requires adaptability to many facilities.

Home Care/ Social Services

 These responsibilities would include infusion therapy; also caseload management and teaching of/and administration of IV therapy to home care patients with HMO-based insurance. Vital responsibilities of home care nursing included initial assessment of patient; status reports and clinical directions, implementation of physicians orders, venipuncture skills, interpretation and reporting of abnormal lab values; recognition of potential patient problems, devising and implementing nursing care plans and completion of 485 Medicare form and updates.
Many of the clients in the medical home care environment at present have HMO based insurance programs. Due to this fact, I must note that I have experience in  Standards of Care with regard to Clinical Pathways that were at times used as a guide for care or the care plan itself. Also, the home care nurse routinely gave reports of patient care for visit approval. I also coordinated care with other disciplines within the home care environment.
I also worked as a visiting nurse of the"   "   County Options program. This program was formerly called  L.A.M.P.  The goal of this program is to assist individuals within the county who are medically eligible to enter a nursing facility to live at home. The county provides a Home Health Aide free of charge. My responsibility was to assess patient needs and the evaluation of the HHA.   Recognizing the social and physical needs (e.g. home equipment) was also a primary responsibility of my work. My primary goal in this environment was to make the home environment as medically conducive to patient health as possible. Many of the clients did not see a
In this regard I also reported any potential or existing problems to the client's doctor.  Since the focus of care is not only on the patient's medical problem, but also on the supervision of the aide and client environment, I also worked along side of social workers who assisted in the needs of the patient.

SPECIAL SKILLS:
Venipuncture and use of EKG monitoring, IV therapy, (maintenance and starts) care planning, completion of 485 and update forms.  I am proficient in use of Excel and Microsoft Word programs.
