If you job says to send in a sample or something, put that in too.
Website(s) You've worked in or owned. (The address to it, the name is not needed)
Comments/Suggestion

Once you email me, I will look it over, and see if you qualify. If so, I will email you back and tell you. If you choose a nickname, you will have to go by that. So don't pick one, if someone else already has it, or you don't want one.
