REASON FOR REFERRAL:
Victoria was referred for an evaluation by her pediatrician, Dr. Diane DXXXXXX, to determine if underlying learning and attentional problems are adversely effecting her academic achievement. Parents are observing problems with Victoria's acquisition of reading skills, her concentration, and her level of cooperation. Her reading tutor suspects possible attentional problems. A full battery of tests was administered to gain current information about Victoria's cognitive abilities and academic achievement in the areas of reading, math, written language, and general knowledge. A parent questionnaire was completed to gain information about Victoria's social, emotional, and attentional factors.
