By clicking the above button, the "payer" also known as the "client"
agrees to the following terms by submitting payment, and understand the following terms
without doubt or question prior to the "payee" also known as the "technician" performing
requested services.
I, “payer”, also known as the “client”, authorize, you, “payee”, also known as the
“technician”, to perform the services and/or repair indicated above and furnish any
necessary materials. I understand that any cost is an estimate, and the final bill may be
different and subject to applicable taxes. Your technician may operate the computer for
diagnostic, repair, and testing purposes. I hereby acknowledge that during the course of or
as a direct or indirect result of the actions or omissions taken by the technician, to
repair or otherwise service my computer system, including but not limited to, the hard
drive, monitor, and any and all software or other hardware, data may be irretrievably lost.
I hereby acknowledge that the technician shall not be liable, or responsible, agree to
indemnify, hold harmless, and defend the technician, either in tort, contract, or
otherwise, for any such loss of data and/or equipment or for any incidental, indirect,
punitive, exemplary, special, or consequential damages related to or arising from such loss
of data and/or equipment. I agree that you will not be responsible for loss or damage of
data and/or equipment in case of fire, theft, or any other cause beyond your control. I
understand that an express technician lien is acknowledged on the above equipment to secure
the amount of repairs and/or service. I own or am otherwise licensed and/or authorized to
use the data and/or equipment including any and/or all software provided, and authorize the
technician to use it to install, repair, or otherwise service my computer system, and agree
to indemnify, hold harmless, and defend the technician against any claims, demands, losses,
or actions resulting from the technician use of such software and/or equipment to install,
repair or otherwise service my computer system.
