Please download the "ISF Student Application " forms.
Send a check in the amount of $150.00 plus the completed application to:

Download all the forms and make a $150 check to ISF application:
Students must have the following forms on record with the ISF. Students need
to turn in these forms to their coaches:
(A) Student Application
(B) Liability Waiver
(C) Emergency Medical Release
(D) Code of Ethics
Coaches need to fill out the following forms: COACH'S APPLICATION FORM
(A) School and Coach Information Sheet
(B) Coach's Agreement (available in Section II of the Coach's Handbook.
(C) CPR & Basic First Aid (coaches must send a copy of their CPR and Basic
Certification cards to the ISF office each year)
