Note-

All forms are in Adobe Portable Document Format (PDF) and and in html forms. PDF forms can be filled in on the screen, printed and should be brought to me. You will need the free Adobe Acrobat Reader version 4.0 or higher to view, fill and print correctly. If you have any problems viewing or printing a form, please check the Adobe Reader Support website.

If you don't have Adobe Acrobat you can simply email  me the necessary information.

Membership Forms Information-

Each year a membership form must be filled out. New members should fill out the interest form, returning members should fill out the returning member form. Everyone must read the guard membership information packet and have the consent form filled out and signed prior to auditioning for The Colorguard.

Leadership Application-

Anyone wishing to be considered should fill out the leadership application. You may also enclose recommendation letters and a grade check form with your application.
