1. I certify that the above-described Labradoodle is of sound health at the time of sale. Said puppy comes with a 2-year genetic health guarantee for PRA (eyes) and Hip Dysplasia. We will replace the dog if any of the above mentioned occurs with a dog from another litter of equal value, when available at no charge other than shipping or 50% of the purchase price will be refunded. 2. We must receive written proof from a licensed DMV. We also retain the right to have a 2nd opinion at our expense. The buyer must have puppy examined by a competent veterinarian of their choice within 3 days of the date and time of delivery for a well puppy exam. 3. Current age appropriate vaccinations, worming, Micro-chipping, Spay/ or Neutered, medical records, Registration from the International Labradoodle Association and a puppy pack from Canidae with sample puppy food. Initial_________

4. If within 3 days of this date, the puppy should become ill, you may return the dog and get a full refund or a new dog. However if buyer takes the puppy to the vet, during this period or incur any expense, the seller will not be responsible for any vet bills unless prior consent is given in writing. 5. Your new puppy will have been seen by a veterinarian, have a health check up with a 100% clean bill of health, appropriate shots and be wormed prior to your receiving the puppy. Initial________

6. If you would like to reserve a puppy we take a $350 non-refundable deposit on a male or female puppy. First come first serve, such as -- first deposit gets first pick of the litter, second deposit gets second pick, third deposit gets third pick. And so on, you get the idea. Final picks begin when the puppy is 4-6 weeks old. We require that the puppy be paid for in full by the 6th week or when your puppy has been picked.
