GENERAL AFFIDAVIT
STATE OF FLORIDA
COUNTY OF HAMILTON
I, John O'Neal, do hereby declare under the penalty of perjury that the following statement is true and correct and made of my own free will, and based on my own personal knowledge:
On April 1, 2001 as the yard closed, around 7:30 p.m., I entered F Dorm for count. Upon that time Sgt. C.O.II Sheddan asked for my id card. At that time I went to my bunk for count. After count, I was called to the
officers station, then I was given a "CC"for
coming in the dorm late for count. At that time Officer C.O. Gary ordered me to go to my bunk. About 12 minutes later, I requested to use the
bathroom, at that time I was told to go the laundry room gate by Officer C. O. Gary. I was let inside of the gate and there I waited until C.O. Gary called Sgt Sheddan, once Sgt Sheddan arrived, he and C.O. Gary was talking
while my back was turned away from them, and all of a sudden, I felt a hard force push and [someone] shoved me into the shelf. I looked up and it was Sgt. Sheddan. I then requested a medical emergency cause my
whole side of my ribs on my left side was
hurting real bad, Sgt Sheddan took me to
medical and I was trying to explain to the nurse what happened, but I was scared, cause Sgt Sheddan was inside the nurse's office while my wounds were being looked over. I could
not tell her he did it cause my life I felt was in danger. This is an aggravated assault, battery by Sgt Sheddanin the presence of Officer C.O. Gary who
started this conflict. She is a part
of this assault. I file this complaint for charges to be filed. Medical has records.
I have read the
foregoing General Affidavit and declare under the penalty of perjury that it is true and correct pursuant to Florida Statute
92.525(2) in lieu of a notary this 5th day of April , 2001.
