CHILDCARE CONTRACT |
|
|
DURATION OF CONTRACT:
By signing the childcare agreement, you will be subject to the terms
and conditions listed below. Your contract will be evaluated yearly and whenever your child reaches the
next age level (infant, toddler, preschool), and will be subject to the specific terms, conditions and
fees in effect at that time.

|
NOTICE OF CHANGES:
BY PARENTS: Parents are required to give three weeks written notice
should they wish to change or terminate the contract. An equivalent fee for care will be charged if insufficient
notice is given. Unused vacation days may not be used in lieu of fee payment.
BY PROVIDER:
Provider will also give three weeks written notice should they wish to change or terminate the contract.
The provider reserves the right to terminate without notice for breach of the terms and conditions of
this contract or if, in the provider's opinion, the continuation of the child or parent as part of the
daycare group is detrimental to the well-being of the other children in care.

|
REGISTRATION FEE:
An initial administration/set-up fee of $50 for the first or only
child in a family and $25 for each additional child is due at the time the contract is signed and is
non-refundable. Your child's space in my daycare is not secure until this fee has been paid as it is
part of the registration process. The fee is used to cover the interview materials, and to set up the
supplies your child will need to start care as well as the preparation of name specific items such as
our wall displays. The balance is used to purchase essential equipment.

|
PAYMENT OF FEES:
Fees are charged on a weekly basis at the rate set out in your signed contract.
Fees are due by cash or cheque each Monday prior to care. This reserves your child's space in my daycare
for that week. Any additional fees incurred for extra hours, meals, or services should be included with
the next payday. There is no reduction or rebate for hours or days not used. Please remember that your
childcare fees are my income. Your child's meals and learning activities depend on them.
LATE
FEES: A late fee of $5 per day will be charged for each day that your childcare payment is late.
This includes Saturday and Sunday. Fees not paid by Friday will jeopardize your child's space in daycare
for the following week. After two weeks of fees plus late payments are in arrears, I reserve the right
to terminate care without notice. Unpaid fees and penalties are subject to legal action.
NSF
CHEQUES: Parents will be notified of any cheque returned NSF. The late fee will aplly from the date
the cheque was not able to be posted to my account (date on letter accompanying returned cheque or date
posted in my transaction book - whichever is earlier) until all fees owing are paid in cash. Parents
will also be responsible for paying any fees I incur as a result of the NSF cheque. Following an NSF
incident, all future payments must be made on time and by cash until I reinstate cheque privileges.

|
RECEIPT FOR PAYMENT:
A receipt will be available following each pay period. A yearly childcare
statement for taxation will be given. Fees for a duplicate receipt are $2 for a weekly statement and
$5 for an annual statement.

|
ABSENCE FROM CARE:
Parents are required to let the provider know as promptly as possible of
any day that your child will be absent or late. Failure to arrive within 15 minutes of your regular
drop-off time could jeopardize care for your child. There will be no fee compensation. I have a responsibility
to the other children to maintain a normal schedule of meals, snacks, walks to the park, school drop-off/pick
ups, etc. A vacation day absence can only be utilized if I receive a call within the 15 minutes of your
regular drop-off time. Otherwise, fees will be due. Advance notice is appreciated but I realize you won't
know about absence due to illness until early morning. I will also notify you as soon as possible if
I will be closed for the day due to illness or family responsibilities. Please have a back-up plan for
care.
CHILD ABSENCE: July/August: Fees will not be charged for up to 10 days of summer
vacation. Additional summer days/weeks may be taken with half the regular fee due. To utilize the half-fee
rate a minimum of 24 hours notice (one full daycare day) must be given.
September-June: In addition, from September to June, parents are entitled to an additional
10 days for full-time and 5 days for part-time when fees will not be due. Additional days during this
period will require full payment. These days are designed to be used when you have a day from work, child
sick days, appointments, holidays, or relatives visiting. They may not be used for Statutory holidays
when I am closed anyways. Unused days may not be carried over to the next year and may not be used during
the final three weeks of care before termination. It is your responsibility to notify me that you are
taking a vacation day and to sign the vacation log. The fee rebate will be deducted from the following
weeks payment.
PROVIDER ABSENCE: Fees will not be charged for days
the provider is unavailable for care (with the exception of Statutory Holidays-see below). Written notice
will be posted on the parent board when possible. Parent is responsible for arranging back-up care.

|
STATUTORY HOLIDAYS:
The daycare will be closed all days recoginized by the Federal Government.
(ie. If my husband doesn't have to work then I am closed too.) These are paid holidays and fees will
be charged accordingly. Exceptions will be discussed in advance. Depending on family plans, care may
be available, at my discretion and for an additional fee, to any parent who doesn't get the day off.
Notice of early closings (such as Christmas Eve) will be posted on the parent board in advance.

|
EQUIPMENT:
Most equipment needed by your child during a regular daycare day will be supplied
by the provider. This includes such items as highchairs, playpens, toys. Parent is responsible for supplying
child's carseat should the need arise.

|
|
CLOTHING:
Each child should have at least one complete change of clothing including socks
to be left at the provider's house. Child should come prepared for outside play according to the season.
Extra hats, mitts, sunhats, etc. can be stored. Your child will require a pair of Velcro closure running
shoes to be left at my house for indoor use only. Slippers and sock feet are dangerous on hardwood floors.

SPECIAL NEEDS:
Should your child have special needs, either physical, mental or dietary, it
is important to discuss the details with me and what the expectations for that child should be. The fee
for a special needs child will be based on the level of care required and will be raised or lowered accordingly
as skills are accomplished or milestones are passed. Parent is responsible for sending any special foods
or equipment required because of a medical condition. Written material detailing the special needs is
appreciated.

|
ILLNESS:
A child is too sick to attend care when they are feeling too uncomfortable to participate
in our regular schedule. A child with vomiting, diarrhea, rash or seeping sore of any kind, should not
be brought to care. Mild colds are acceptable but children with exessive coughing or a constant nasal
discharge should be kept at home. Any child with a fever over 101'F or 38.5'C, in the past 12 hours,
should not come to care, and especially if the child's temperature can only be maintained at a lower
rate by the use of fever medication. This indicates a sick, potentially contagious child who would be
uncomfortable at daycare because our activity level would be detrimental to controlling the temperature.
Parents may utilize a vacation day during a child's illness if they have unused days provided I am notified
within 15 minutes of the regular drop-off time that the child will be absent. Otherwise, full fees
are due for any absence from care. For the health and safety of the other children in care, I reserve
the right to refuse care to any child that appears ill. If a child becomes ill during the daycare day,
the parent will be required to make arrangements to pick up the child promptly.

MEDICATION:
The provider will administer prescription and non-prescription medications (at
my discretion) according to written instructions from the parent. A copy of the medication form can be
found in the parent folder. Please photocopy several so you have tham handy when necessary. Unless I
have a written request detailing otherwise, I will assume that I have your permission to the use the
following products on your child with brand of my choosing: baby wipes, diapers, diaper cream, first
aid creams and products, moisturizing lotions, sunscsreens. This list is not exhaustive.

DROP-OFF/PICK-UP:
Parents may only use the hours scheduled in their signed contract. Any changes
must be approved ahead of time. Do not assume I am available for extra care outside these hours. I plan
my own family's activities according to the contracted schedule. Feel free to ask but please don't be
offended if I ask you to make alternate arrangements if you must work early or late. Children not arriving
within 15 minutes of their sceduled drop-off time jeopardize the availability of care for that day. Late
fees will be due for each hour or part hour for children not picked up on time.

RELEASE OF CHILD:
Child will only be released to someone whose name appears on the contract
forms. Should it become necessary, due to an emergency, for someone else to pick up your child, I require
notification by phone of the change, the name, address and contact phone number of the designate. The
person will be required to produce a driver's license with name, address and photo verifying their idetity.
Alternate photo ID will be considered. If you know in advance, written instructions are the best way
to protect your child. By law, I must have a copy on file, of any court imposed restrictions regarding
access. I realize these restrictions can pose a hardship for parents with demading jobs or social schedules
but it is done for the protection of your child. Just for a moment, imagine what the consequences could
be.
