Fuel charge----------$30.00 (subject to change due to cost)
Delivery charge------$50.00(No delivery fee for single trip of 5 yds or more)

Hot Water-----$2.50 per yard(supplied Oct. 15th - April 1st)
Dump Truck for hire--------$50.00 per hr. plus disposial fees for removal.
Normal delivery schedule M-Sat 8am-5pm

Unloading time: Ten (10) minutes per cu. yd. Standby @ $75.00 per hr.
Cancellation: 4 hr notice (except for weather- 2hrs) Show up, no delivery--$100.00
Deliveries beyond area: $35.00 15 mile radius 3 yd minimum.
All deliveries will be COD.
All invoices must be paid upon delivery unless PRIOR arrangements are made.
No claims for delays encountered in delivery will be allowed as all deliveries are made to the best of our ability and dispatch.
I, the customer, will assume all responsibility for any damage where delivery is made inside of the curb.
Use of this product without protective clothing could cause bodily injury. Every effort should be take to prevent contact with skin.
The balance is due and PAYABLE UPON DELIVERY. Or reciept of invoice only if PRIOR arrangments have been made. After 15 days a LATE CHARGE OF 2% monthly (ANNUAL PERCENTAGE RATE OF 24%) will be added to the unpaid portion of the balance due. I agree to pay and/or guarantee payment of the charges listed on the invoice when due. I agree to pay reasonable attorney's fees and court costs incurred by BOISE SHORTLOAD CONCRETE, Inc., in any bankruptcy proceeding in collection of the balance due weather or not action is filed in court. I agree that the liability is being personally assumed by me unless otherwise indicated on the invoice and the agreement does not constitute release of liability. By my signature on the invoice, acknowledgment and agreement of the above is hereby made.
BOISE SHORTLOAD CONCRETE, Inc. hereby gives notice of its reservation of the right to assert any and all liens against the purchaser and third parties arising under all applicable federal, state and local laws in the event of the prices stated are not paid when due.
I reviewed and approved a complete copy of the invoice.
BOISE SHORTLOAD CONCRETE, Inc. agrees to provide, to the best of their abilities, all services and materials indicated on the invoice.
