Dear Colleagues/Patients,

It is with mixed emotions that I inform you that I am reducing my private practice hours effective March 1, 2005. But I am glad to inform you that I will be increasing my hours working at the Veteranís Administration Hospitals and Clinics, supervising and teaching medical students and post-graduate physicians.

For my remaining office patients, I will only be available after 5 pm and by appointment only. I will accept a few referrals for electrodiagnostic studies (EMG/NCS) by calling my voicemail, (818) 993-3151, vmb #1. Please give me the patientís phone number and Iíll return the call and schedule the appointment.

For all other PM&R referrals, consultations and pain management, I have several colleagues that I can recommend. Dr. Robert Galbraith shares my office space in Northridge and can be reached at (818) 993-3151, vmb #2. And there are now several other private practice physiatrists serving the San Fernando Valley including Dr. Glenna Tolbert at (818) 784-7197, Dr. Behzad Emad at (310) 721-6162 and Dr. P. Eli Shirazi at (818) 789-3964.

I was planning a gradual transition but I was offered an opportunity that I could not postpone. Please accept my apology and Iíll be glad to help in any way I can.
