How to retrieve a TDC membership application

The TDC membership application has been converted to an Adobe Portable Document Format (PDF) file. In order to see and subsequently print the form, you will need to have Adobe Acrobat ®: Reader ® on your computer. This application is free to download from Adobe. After you download the membership application to your computer, print it. Then complete the information legibly. Mail the application with a check payable to TDC for the required fees to the address shown on the bottom of the form.
