Well, Chibi-Chibi is two completely different people. We will first state her anime story, and then her manga one.
In the anime, Chibi-Chibi comes flying from the sky one day, holding onto an umbrella. Usagi finds her and her family takes her in. The only thing she can say is her name, except in episode 200. Chibi-Chibi comforts Usagi when all her friends are dead and she helps her defeat Galaxia.
But who is she? It is not until the final battle that we learn her origin. You see, there are Sailor Senshi in other parts of the galaxy too. The strongest and bravest of all these was Sailor Galaxia. She had saved many people, and had a pure soul. Then an evil force began spreading across the galaxy, Chaos. Galaxia decided that the only way to try and stop it was to seal it inside herself. But she knew that would not fully end it. She sent her star seed(soul, light of hope) out into the galaxy and told it to find a place where it was loved. That starseed was Chibi-Chibi.
When Chaos had turned Galaxia into an evil enemy, she began to conquer the galaxy with her Sailor Animates. In episode 200, all of Usagi's friends are dead, and Usagi is the last hope for the universe. She is terribly sad at the loss of all her loved ones, and Chibi-Chibi cheers her up,turning into Sailor ChibiChibiMoon. Sailor Moon's star seed is not strong enough, but Chibi-Chibi helps her.Chibi-Chibi transforms into a sword and gives Sailor Moon encouragement to destroy Galaxia. In the end, Galaxia is healed back to her good self. However, what was done cannot be undone, and Galaxia and Chibi-Chibi go into the heavens. Chaos went back where it belonged, into everyone's hearts, and to combat it, the Light of Hope went into everyone's minds.
In the manga, Chibi-Chibi is someone completely different, so don't confuse this with the anime story. You see, in the manga, at the end of her final battle, Eternal Sailor Moon must throw herself into the Cauldron of Chaos in order to completely destroy it. However, a tiny speck remained and grew into a menace. Sailor Cosmos, Sailor Moon's future future self, is too cowardly to fight it. She keeps running and finally it is too big to fight. She decided she must go back into the past and help Eternal Sailor Moon completely defeat it. She does not go back as herself, but as Chibi-Chibi. Sailor ChibiChibi Moon can transform by herself in the manga, talk, and look like a regular senshi, only a bit shorter. She comforts Usagi when she is sad, just like in the anime.
In the last battle, Sailor Moon is about to throw herself into the Cauldron and seal Chaos when Sailor Cosmos appears. She tells her her story, and offers to help seal Chaos in. Sailor Moon considers it, and decides not to accept her help. You see, Sailor Moon understands that the joy of life is having the new evils along with the new joys. If you believed in the hope of good things, you could conquer the bad. Sailor Cosmos finally understands, and goes back into her time to fight Chaos, as is her destiny. Sailor Moon sacrifices herself to seal Chaos, but somehere in the flow of lights, she is reborn on the morning of her wedding day.
Sailor ChibiChibi Moon has no attacks, but she gives Sailor Moon one-Silver Moon Crystal Power Kiss. She also is strong and has powers to heal and help.
