Checklist for Intervention

 

You’ve noticed that one of your students is having particular difficulties in one or more areas…

 

Did you make and record observations either within specific scenarios or across situations?

 

Did you assess the student’s strengths and needs?

 

Did you discuss your concerns with a previous teacher?

 

Did you review information from the student’s OSR?

 

Did you make initial program adjustments?

 

Did you contact parents to discuss the matter and explain your planned adjustment?

 

Did you observe and assess the impact of your adjustments on the student’s learning?

If the difficulties persisted…

 

Did you make a referral to the in-school team?

 

Did you continue to communicate with parents?

 

Did the in-school team plan a meeting to discuss your concerns and invite the parents, the principal, the special education resource teacher and any other professionals who could provide pertinent information?

 

Did the team determine any interventions or accommodations to address the student’s areas of need?

 

Did the team take the preferences and interests of the student and the parents into account when making decisions?

 

Was a plan for in-class accommodations created?

 

Were additional supports or withdrawal for remediation or enrichment decided upon?

As the classroom teacher, have you taken the expectations from the student’s IEP as your guide in planning instruction for this student?

 

Have you made modifications to the quantity of work at grade level required?

 

Have you made modifications to the grade level expectations?

 

Have you assessed the student based on these modified expectations?

 

Did you continue to monitor and record the results of these actions and review their efficacy after several weeks?

 

Did the in-school team make a referral for any other specialized services such as medical, speech/language, psychological, social, vision or itinerant hearing services?

 

Was a referral for assessment from any of these professionals made?

 

Did the team get parental consent for testing?

If the accommodations and interventions were implemented, but found insufficient…

 

 

Did the in-school team or the parent decide that an IPRC meeting was necessary?

 

Have the ongoing and continuous interventions been monitored and the results recorded for review and discussion at the IPRC meeting?

 

Has an educational assessment been completed?

 

Have additional assessments or tests been completed?

 

Have all necessary parties been informed of the IPRC meeting?
